Chapter 3 - The Threat Behind the Smile

Hook:
Dr. Vance didn’t flinch; his smile just solidified into stone.
Conflict:
“Routine evening sedative, Leo,” Vance said, stepping past me toward the IV pole. “Her intracranial pressure has been spiking intermittently. You know how dangerous autonomic storms are for brain-damaged patients.”
“The charge nurse usually handles the medication runs,” I said, keeping my body anchored squarely in his path. “I haven’t seen an attending physician push an IV flush at 8:30 PM since she was admitted.”
Discovery:
Vance’s eyes narrowed slightly, a subtle twitch flashing along his jawline. “Because she’s my friend’s sister-in-law, Leo. Because I promised Mark I’d give her concierge care. I don’t leave critical neurological patients to overworked floor nurses.” He pulled the vial out, revealing a clear liquid. The label was peeled clean off. “Now, do you mind stepping aside so I can administer this before her vitals fluctuate?”
Twist:
From the bed, a monitor chimed sharply. Sarah’s heart rate spiked from 68 to 112 beats per minute. The digital numbers glowed red on the screen.
Toby whimpered, gripping my belt loop. “Dad, look at her hand.”
Under the white sheet, Sarah’s right hand was balling into a tight fist. She wasn't just spasming; she was fighting the air, her knuckles whitening with desperate, terrified strain.
Emotion:
I felt a wave of hot, electric rage surge from my gut straight to my temples. My wife wasn't brain-dead. She was conscious, trapped in the prison of her own paralyzed nervous system, terrified out of her mind of the man standing two feet in front of me.
“Hold off on the injection tonight, Robert,” I said, my voice steady, dangerously quiet.
“Leo, with all due respect, you're an architect, not a physician. If she herniates her brainstem because you skipped an anti-spasmodic, she dies tonight.”
Cliffhanger:

“I said hold off,” I repeated, resting my hand heavily on his forearms, feeling the tense muscle underneath. “In fact, let’s get the Chief of Medicine down here to sign off on that unlabeled vial right now.”
